A puck screen is a thin mesh or perforated metal disc placed on top of the tamped coffee before brewing.
What it does
Distributes water more evenly across the surface of the puck, rather than letting it arrive in jets from the shower screen holes.
Protects the puck surface from being disturbed as water arrives, which can reduce surface channelling.
Keeps the shower screen clean. Coffee does not migrate up into it, so cleaning is much easier. Many people buy them for this alone.
Gives a drier puck on some machines, since less water sits in the headspace.
Does it improve the taste
Modestly and inconsistently. Careful tests show small improvements in evenness, and it is not transformative.
It matters most on machines with poorly designed shower screens and on setups with headspace issues.
What it does not do
Fix channelling. Channelling happens inside the puck, from uneven density, and a screen on top does not address it.

The practical considerations
Thickness. A thick screen adds height and may cause the shower screen to press on the puck. Reduce the dose slightly if so.
Cleaning. They clog with oils and need regular soaking in detergent.
Worth buying
For the clean shower screen, yes — it is a real convenience.
